Montgomery Family Medicine is currently accepting new patients however there may be a waiting period for your first appointment depending on the physician's schedule. Please call our office for help in getting your first appointment.  Get an early start by printing out the new patient forms below.

Providing medical care requires a certain amount of paperwork.  Prior healthcare data, insurance information, release consents  just to name a few.  Here we try to provide you some of those forms we will be asking from you at your first visit.  Some of these forms are required by law, all are needed in order to ensure you the best healthcare delivered as efficiently and cost-effectively as possible.
