You can make an appointment by dialing 396-9100 between the hours of 7:45 AM and 4:30 PM Monday through Friday. Detailed directions are provided below.  Our phone nurses schedule appointments. Please arrive on time for your appointment and we will do our very best to see you on time.  All new patients should arrive twenty minutes early to complete in processing.  PLEASE do not forget to bring your insurance card.

Appointments are usually not available the same day, but we will make every effort to see you. We will schedule you as a work-in if your injury or illness warrants it. Work-ins may be asked to come in and wait for an available physician. As a work-in, you may not be able to see the physician of your choice. As a group of physicians, we are all committed to caring for all the patients of the practice and we are all equally capable.

Please be sure to bring your insurance card and your driver's license with you to your appointment, we will make a copy to keep in your file.

Montgomery Family Medicine is opened Monday through Friday 7:15 am to 4:00 pm.  We are located in Sturbridge Commons at 8190 Seaton Place near the corner of Vaughn and Taylor.

From I-85, take the Taylor Road-AUM exit (number 9) and turn right (South).  When you get to Vaughn, turn left.  Turn right at the second traffic light (you are now entering Sturbridge Commons).  Take your first left and we are at the end of the street.
